Frequent questions

What Insurances do you take?

We only take PPO insurance, no HMOs or Denti-CAL. We are in-network with most PPO insurances, but not all. As a courtesy, we will have a break-down of your insurance benefits before your appointment and create a treatment plan based on this.

What do I need to make a New Patient appointment?

If you are a new insured patient, we need the following:

  • Patient Name and Subscriber Name (if different)

  • Patients DOB and Subscriber DOB

  • Insurance Carrier and Subscriber ID or ss#

  • Email and telephone

What do I need to bring on the day of my first appointment?

We need a form of photo ID, and the New Patient forms filled out before your appointment.
